These Terms & Conditions ("Terms") govern your access to and use of the website located at joinnoura.com (the "Site") and the services offered by Noura Health ("Noura," "we," "us," or "our"). Medical services are provided by Dr. Raina Gazurian, MD, and her medical practice. By accessing the Site or enrolling in our services, you agree to these Terms. If you do not agree, please do not use the Site or our services.
1. WHO WE ARE AND WHAT WE OFFERNoura Health is a concierge, cash-pay practice for women 35+. Through Noura you may access:
- Medical care — full-scope concierge primary care, hormone and perimenopause/menopause care, ADHD evaluation and treatment, metabolic health, and integrated mental wellness, provided by Dr. Raina Gazurian, MD.
- Coaching and wellness support — ADHD coaching, nervous-system regulation, mindfulness, and behavior-change coaching provided by Danielle Taylor, RN.
Medical care and coaching are distinct services. Coaching is not medical care, mental-health treatment, psychotherapy, or a substitute for care from a licensed clinician, and no provider-patient relationship is created by coaching alone.
2. WHERE WE PROVIDE SERVICES
Noura provides primary care and hormone care to residents of the states in which the practice is authorized to operate:
Pennsylvania, New York, New Jersey, Connecticut, Virginia, Florida, Washington, California, and Kansas. ADHD diagnosis and prescribing are
currently available in Pennsylvania only; ADHD-informed care and coaching are available in all nine states. Services are offered virtual-first, with in-person visits available to members in the Doylestown / Bucks County, Pennsylvania area. We may add or remove states at any time.
3. ELIGIBILITY
You must be at least 18 years old and a resident of a state where we are authorized to provide the service you are requesting. By using our services you represent that the information you provide is accurate and that you are legally able to enter into these Terms.
4. NOT FOR EMERGENCIES
Noura does not provide emergency services. Our care is not appropriate for medical emergencies. If you are experiencing an emergency, call 911 or go to the nearest emergency room. If you are experiencing a mental-health crisis or thoughts of self-harm, call or text 988 (Suicide & Crisis Lifeline) or go to your nearest emergency room.
5. MEMBERSHIP TERM, RENEWAL, AND CANCELLATION
Noura membership is offered on a
one-year (12-month) term. Unless otherwise stated in your enrollment agreement:
- Your membership begins on your enrollment date and continues for twelve (12) months.
- At the end of the term, membership renews automatically for successive one-year terms unless you cancel before the renewal date.
- You may cancel by contacting us at [CONTACT EMAIL] before your renewal date.
- Cancellation stops future billing; it does not retroactively refund fees already paid except as described in Section 6.
- We may decline to renew, or may terminate a membership, where continuing care is not clinically appropriate, where fees are unpaid, or where conduct violates these Terms.
6. FEES, PAYMENT, AND REFUNDS- Noura is cash-pay. Membership and coaching fees are described at enrollment and are due as scheduled.
- We do not bill insurance for membership or concierge services. Your labs, imaging, and prescriptions are ordered by your physician and run through your own insurance at your usual lab or pharmacy. On request we provide an itemized superbill you may submit for possible out-of-network reimbursement; we do not guarantee reimbursement.
- Coaching is billed per session: $75 per session for Noura patients, $180 per session for non-patients. Pricing is subject to change with notice.
- Fees are non-refundable except where required by law or as expressly stated in your enrollment agreement. Specific refund terms in your enrollment agreement control over this section.
7. PRESCRIPTIONS AND MEDICATION
Prescribing is at the independent clinical judgment of your physician and is never guaranteed. We do not prescribe on demand. Where weight-related or metabolic medication (including GLP-1 medications) is clinically appropriate, we use only
FDA-approved, brand-name formulations through regulated pharmacies, never compounded. Controlled-substance prescribing, including for ADHD, is provided only where clinically appropriate and only in states where we are authorized to prescribe (currently Pennsylvania for ADHD).
8. TELEHEALTH
Many services are delivered by telehealth. Telehealth has benefits and limitations, and not every condition can be safely evaluated or treated remotely. Your provider will tell you if an in-person visit or referral is needed. A separate telehealth informed-consent may be required before your first visit.
9. YOUR RESPONSIBILITIES
You agree to provide accurate and complete health information, to follow the care plan you agree to, to keep your contact and payment information current, and to use the Site and services lawfully. You are responsible for maintaining the confidentiality of any account credentials.
10. NO GUARANTEE OF RESULTS
Health, coaching, and wellness outcomes vary. Nothing on the Site or in our communications is a promise or guarantee of any particular result.
11. INTELLECTUAL PROPERTY
The Site and its content — including text, copy, graphics, logos, the Noura name and marks, "Renewal, in practice.," and related branding — are owned by Noura Health or its licensors and are protected by law. You may not copy, reproduce, or use this content without our written permission.
12. THIRD-PARTY SERVICES AND LINKS
We use third-party tools (for example, scheduling, intake forms, payment processing, and messaging platforms) and may link to third-party sites. We are not responsible for third-party content, services, or privacy practices. Your use of those services is governed by their own terms and policies.
13. DISCLAIMERS
The Site and services are provided "as is" and "as available." To the fullest extent permitted by law, we disclaim all warranties, express or implied, including merchantability, fitness for a particular purpose, and non-infringement. Information on the Site is general and is not a substitute for individualized medical advice.
14. LIMITATION OF LIABILITY
To the fullest extent permitted by law, Noura Health and its owners, members, and personnel will not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the Site or services. Nothing in these Terms limits liability that cannot be limited under applicable law, including liability for professional medical care, which is governed by applicable medical-malpractice and licensing law.
15. INDEMNIFICATION
You agree to indemnify and hold harmless Noura Health and its personnel from claims arising out of your misuse of the Site or services or your violation of these Terms.
16. CHANGES TO THESE TERMS
We may update these Terms from time to time. The "Effective Date" reflects the most recent version. Continued use of the Site or services after changes take effect constitutes acceptance.
17. GOVERNING LAW AND DISPUTES
These Terms are governed by the laws of the
Commonwealth of Pennsylvania, without regard to its conflict-of-laws rules. The parties agree that any dispute arising under these Terms will be resolved in the state or federal courts located in Pennsylvania, and you consent to their jurisdiction.
